PHOTO, SOCIAL MEDIA, & VIDEO RELEASE
I hereby give my consent to Black River United Way and Andrews Elementary Kinder "Garden" Program to use my child's photography, video, audio, and likeness in all of its publications including its website(s), social media outlets, public services announcements, marketing materials etc. I hereby waive any expectation of compensation for photos/videos that I approve of publication.
